2021-2022 Budget Request FormPlease submit your budget request for the current or upcoming school year. All budget requests must be approved by the Conn PTA board and by at least 20 general PTA members each school year. If the budget request is for the current school year, the request cannot be granted without the necessary approvals unless funds are readily available within the approved budget. This form must be completed in its entirety in order for your budget request can be considered. This applies to both NEW and PRIOR YEAR programs. If you do not receive an email confirmation from the PTA Treasurer or Financial Secretary within 48 hours to confirm receipt, please email firstname.lastname@example.org.
